All patients do not have insurance. All employers do not provide coverage for employees. However, the lack of insurance should not prohibit you from having the care you need for your oral health.

LavenderCare provides you with two preventative appointments throughout the year including your cleaning, exam, fluoride, and any necessary x-rays. As a club member, you also enjoy exclusive discounts on non-preventative services. LavenderCare is a worry-free dental subscription option. No maximum, no deductibles, no waiting periods, no concerns about your insurance denying necessary treatment resulting in unexpected out of pocket costs.

What forms of payment are accepted?
Annual memberships can be paid by cash, check, and all forms of credit and debit cards including Visa, Mastercard, Discover, and American Express. CareCredit may be used as well. Monthly plans must be signed up with a credit/debit/pre-paid card as they are recurring monthly payments.

When can I start using my membership?
Your membership is effective immediately!

How do I schedule my preventive care?
Simply call our office and set up an appointment for yourself or your family members.

How can I use my Member Exclusive discount for restorative, elective, or cosmetic treatment?
Simply schedule an appointment with our office and be sure to tell us you’re a member. When you arrive, we’ll make sure your membership is“current” and you will be eligible for instant savings off our regular fees.

How do I know if I need Perio membership?
If you’re a current patient, please contact us to confirm you’re on Perio Maintenance. If you are a new patient, we will make a recommendation after your initial exam.

What happens if I miss an appointment?
It will be your responsibility to reschedule. Please see our office policy regarding no-shows and last-minute cancellations.

Can I add members to my account?
Absolutely! Your entire family can join
 but each patient must have the required plan.

Can I transfer my membership to another dentist?
Your club membership is solely with our office and as such is non-transferable. If you move to another area, you may cancel your monthly membership at any time. Unfortunately, we cannot provide a refund for annual plans.

How can I check on the status of my account?
Login to your Member Portal and look under Account Members. You’ll see the status of each member above his/her name.

Can I change or update my billing information?
Yes! Depending on your dental office membership plan, log in to your Member Portal for monthly plans and click “Edit Your Payment Method” or give your dental office a call to update any annual plans.

How do I cancel my membership?
For monthly plans, log in to your Member Portal and click the “Remove” button for each member you’d like to cancel. Again, we cannot provide a refund for annual plans.

If I cancel, can I get a refund on my membership?
No. Refunds will not be provided for fees paid under any circumstances, including failure to schedule and maintain appointments.

How much do I pay to sign up for my club membership?
You’ll pay only the Lifetime Activation fee to join for monthly membership plans. This fee includes your first month’s membership fee. There is no activation fee for annual plans.

Can I prepay my membership fees?
Monthly memberships are currently paid month-to-month only. Annual membership plans do need to be paid in full at the onset of sign-up. 

What information is required for me to sign up for my club membership?
An email address for receipts, credit or debit card, full name, address, phone and date of birth.
